Our lab has been purchasing masks from Advance Reproductions Corp, and we are pleased with them. Their website is:

A 4 X 4 X 0.090 soda lime coated with chrome cost us $450. This is pricey compared to writing one yourself on the DWL with a blank mask plate, yet it is worth it for us because the improved results are evident in our devices.

The specs they can achieve are 0.75 um for the smallest dimension

The critical dimension tolerance is +/-0.15 um.
* Glass sizes range from 3″ to 32″
*
Substrate materials: quartz, fused silica, soda lime, borosilicate.
* Glass coatings: chrome, iron oxide, aluminum, titanium, gold, and emulsion
* Plate thickness: .020″ to .400″ (10mm)

I can answer part of your question. We buy mask plates from Nanofilm for $20 - $25 /ea for 3"-5" sq Soda Lime plates. The masks are written in-house on a Heidelberg DWL 200.
